Boys: May 1, 2012
Girls: Jan 1, 2012

1. I, the undersigned, desire for my child to play Baseball/Softball during the 2012 season.  I understand that my child’s participation in Linden Little League activities involves risks and dangers of serious and permanent bodily injury, including the possibility of death.  I hereby release, hold harmless, discharge and agrees not to hold Linden Little League, its clubs, teams, directors, officers, employees, coaches, officials and owners/lesser of premises responsible for any injuries that may be incurred during games, practices, transportation, lodging or related social and recreational activities.  In the event of an injury, I am willing for my child to receive first aid treatment.
2. I understand that my child’s participation in this sport is a privilege and not a right.  I understand that participation is conditional and shall continue only upon proper conduct of my child and all other family members at all practices and games.  Improper conduct is that which the individual sport’s operating board or the Linden Little League executive board for any reason deems unreasonable or not beneficial to the team, coaches or community.  Any falsification of any of the above information may constitute grounds for dismissal from Linden Little League activities for the remainder of the season.
3. I understand that pictures, without names or identifying information, of those participating in public Linden Little League events may be posted on the Linden Little League website.
4. I/We agree to provide proof of legal residence (as defined by Little League Baseball Incorporated) and age. I/We understand that our child (candidate) must be eligible under the residence and age regulations of Little League Baseball.
5. I/We agree that our child (candidate) may be required to try out for a team, if such does not attend at least 50 percent of the tryouts, local Board of Directors approval is required for such candidate to be placed on a team.
